Bagi yang sudah diterima selanjutnya WAJIB melakukan daftar ulang dengan cara datang ke SMAN 4 Tasikmalaya pada Jumat, 20 Juni 2025 pukul 08.00-14.00 dengan membawa berkas :
\nBerkas umum
\n1. Fotocopy KK
\n2. Fotocopy Akte
\n3. Fotocopy KTP
\n4. Fotocopy Ijazah\/keterangan lulus SMP
\n5. Fotocopy rapor
\n6. SPTJM asli<\/p>\n

Berkas khusus :
\n1. Fotocopy kartu penanggulangan kemiskinan (khusus jalur KETM)
\n2. Fotocopy SK mengajar (khusus jalur guru)
\n3. Fotocopy surat mutasi (khusus jalur mutasi)
\n4. Fotocopy diagnosis ahli ( khusus jalur berkebutuhan khusus)<\/p>\n

Masing masing 1 lembar dimasukan ke dalam map berwarna biru.<\/p>\n
